Surprise Bill – Provider Is No Longer in Network

I recently got an unexpected bill from my provider for several hundred dollars for a video visit.

This was the first time I’ve seen this provider this year (but saw him many times previous years)

I went to check my EOB and it says the provider is no longer in network.

I know this can happen for a variety of reasons but my question is:

Isn’t there a law that says the provider, or possibly my insurance company, or both, have to inform me that the provider is leaving the network?

Neither my provider or insurance company informed me of this prior to the scheduled visit.

Is there such a law, and if so what is, so I can try to fight this?

I have an EPO plan and I’m in Florida. Thanks.

EDIT: Doesn’t look like there is anything that mandates this. My next question would be about “continuity of care.” Isn’t there a law that covers this and says my insurance company has to let me keep my provider for a certain period of time at the in-network rate? I do have a chronic condition.
