The last name for my health insurance doesn’t match up with my legal name.

I’m on my Dad’s health insurance that he receives through his job until I turn 26. I’m 24 now. The last name on my health insurance doesn’t match up with my legal name. For my insurance, I use my father’s last name.

My doctor just wrote me a referral to get an X-Ray, and he wrote the last name I use for my insurance.

I had an issue a couple of years ago where I went to go get a 24 hour Holter monitor hooked up and the lady looked at my referral and told me that she legally can’t do the procedure unless the names match up. She told me if he would’ve wrote both last names on the referral, it would’ve been cool. She tried to get in contact with my doctor’s office to try to get them to confirm the names and fax it to her, but they wouldn’t pick up the phone.

She had to get one of her managers to give her the green light to still do the procedure.

I forgot to remind my doctor to write both my legal last name and the name I use for my insurance on the referral.

I worry about this happening again when I go do my X-Ray.

What should I do?
