Unexpected pregnancy test ordered during an ER visit

[Texas, BCBSIL, 34F] I went to the ER because I had pain in my lower abdomen and I had bloody stool. I was drawn blood which they told me were for cell count and metabolic panel tests and I had brought in my stool for stool tests. I later received a bill for a pregnancy test in the list of charges that I never consented to nor was it ever mentioned once during my visit.

They did me ask about my last period which was less than 2 weeks before that day, when they were asking me about my weight, allergies, etc, so I did not think anything of it. They did not ask me about my recent sexual history, of which there is none in the last 2 years, so there is NO possible way I could be pregnant (besides, I am gay). The total bill was about $3000, including $800 for the ER visit and $300 for the pregnancy test. Insurance adjusted a $800 discount, but I am responsible for the whole rest of bill to meet my deductible.

Is there any way to fight this unnecessary and undisclosed pregnancy test and if so, how should I go about doing it? Do I talk to my insurance, or do I talk to the physician in the ER? Or any advice on what to write in the email and who to cc for record keeping?

In case it matters, the blood test showed that I had possible inflammation and I was prescribed antibiotics. But a few days later the stool culture came back negative for any disease causing bacteria. So it remains unknown why I had bloody stool and pain, but it had resolved on its own anyway.
