As it turns out UHC changed their vitamin D testing policy 2 weeks before I got it tested as part of a routine physical so it was not covered. I do have a history of vitamin d being between 20-30 (insufficiency) but I’m confused if that counts under the umbrella of “deficiency” for billing purposes. I’m wondering if I should appeal and submit previous blood work showing the insufficiency or if that won’t help because it’s not under 20. I don’t seem to fit any of the other diagnoses covered under the policy.
