What is the best health insurance for middle age (paying out of pocket)

I’m shopping around for insurance for my dad. He is 60 and main condition is mild diabetes/insulin resistance. He has only see a doc once in 30 years for a thorough check up (overseas last year). He has never had health insurance. I’m interested in surprising him with sponsoring an insurance plan for the next few years until he qualifies for medicare (he would never accept me doing this otherwise).

I’m mainly looking for him to get all those general screenings one gets in their 50s and 60s and getting his diabetes care plan tailored appropriately. What should I look for? My budget is around 400$ a month – would that qualify him for a good plan? If so where should I look?
