Which employer coverage would be the best option for me?

I’m being offered employer coverage for the first time in my career. I am 28 years old, in the state of Maryland (though I am a remote worker, so my employer is in another state) and interested in only purchasing it for myself. They are offering Anthem Health Insurance. The first option is a buy-up PPO with a deductible of $3000 and a monthly premium of $262.08. The second option is a base plan CDHP with a deductible of $5000 and a monthly premium of $130.

Next, my employer is offering Delta Dental/Eyemed Vision and HSA. The monthly premium for the Delta Dental/Eyemed Vision is $15. The HSA details state that it is only available if you enrolled in a base CDHP plan and it has a place where you can enter the amount you would like to have deducted from each paycheck.

I had a serious acute medical condition in 2016, but I have recovered and haven’t been to the hospital since 2017. My monthly take home pay is not very high at around $2,000 per month after tax. I would like the health insurance because I was humbled by my unexpected medical condition 5 years ago and I would also like the dental/vision coverage because I wear glasses. I would like the cheapest option, but I also would like to know what the downside of choosing the cheapest option would be. Also, I am not quite sure how HSA’s or any of these plans work. Please explain and offer a recommendation. Thank you.
