Why does the maximum, “Estimated Total Yearly Cost”, vary from plans of the same type (like one EPO vs another EPO) from the same insurance company?

This is confusing because everything I have read says that the different metals (bronze, etc.) don’t effect “quality of care”.

I am comparing EPO plans from FloridaBlue, and I am really having a hard time seeing any major differences in what is covered, like one plan is $15K vs another at $21K total per for the year.

I understand that this is only if you use the plan that you can reach the maximum.

Does it have to do with what they will be willing to negotiate later on with hospitals and pre-approvals? Drugs covered? Thank you
