Pre-certification denied due to pre-condition

I’m using a (pretty crappy) travel insurance and had an accident which the insurance has deemed a “pre-condition”, hence refusing to pay. I am obviously planning to appeal but this takes time, up to 90 days.

My main problem is that the insurance is using this fact to reject every pre-certification for followup treatment I urgently need. I thought pre-certification was used to review medical necessity, not coverage. Is there anything I can do to get the pre-certification accepted asap, and deal with the coverage issue and appeals later?
