Supplemental insurance for out of network?

My options are weird this year due to cobra. They all have the same premium, even though one is an hsa plan and one is a ppo plan with a low deductible and extra fancy benefits involving an employer hospital network. So the ppo plan could save me thousands and would be the clear winner based on the in network numbers, but there’s just one problem. It doesnt have any out of pocket cutoff for out of network coverage. The hsa plan has a $10,000 pocket. But the ppo is just 40% coinsurance infinitely. This frightens me thinking about some worst case scenario where I have a million dollars in out of network bills (is this a real concern?).

What should I do about my out of network coverage? Should I bite the bullet and get the HSA plan? Should I just forget about it as a non existent issue and get the PPO plan? Or is there an in-between option with some kind of cheap supplemental coverage that I could add to my PPO which covers these catastrophic out of network costs that probably wont occur?
