Dispute with LabCorp

I need some guidance.

I had a adult well visit in Dec 2020. My provider drew blood on-site and sent it out to a lab.

Last week I received a bill from LabCorp in the amount of $278. I thought that is odd. I reached the OOP max in Oct 2020.

I checked the EOB for the date of service and I see “your total cost: $0.00” with a footnote. I see a lab charge of $278. Due to your lab max amount $278. In the column where it breaks down what I owe, I see: Copay $0, Deductible $0, Coinsurance $0, Services not covered $0. This is what the footnote reads:

337: This care is considered investigational or experimental. We sent the doctor a letter explaining why. If the doctor/facility is in the plan, the member isn’t responsible for this amount. If the doctor/facility isn’t in the plan, the member may be responsible for this amount.

Question: Both the doctor and Labcorp are in-network. The EOB says my out of pocket is $0 so why does LabCorp insist I owe them?
