Is this insurance fraud?

I go to the dentist at a large corporate chain that accepts my crappy DHMO. They charged me more than the contracted rate for a filling. I have already paid it twice in the last few months and just found time today to come here to talk about the issue (you cannot get anyone on the phone). When I came, the manager said the billing was actually incorrect and changed the bill to show that pulpal treatment was actually the cost instead of the filling (which is not covered by the DHMO). Pulpal treatment was already listed on the bill, but she just moved the amounts to match what I paid being for the pulpal protection instead of going towards the filling which is set at the contracted rate. Is this legal? Do I have any recourse? What would you do in this situation?
