My Fiance’s Doctor Left, and They Assigned Him a New Doctor- Who Billed Him $400 for Being Out Of Network, When He Had No Idea

I don’t know all the health insurance laws, but I know when I had Medicaid long ago, the doctors HAD to tell you if they did anything that was not covered by your insurance. I know it is not the same for regular health insurance, but it seems really crumby that they would switch him to a new doctor without tell him that the new doctor is out of network even though their facility is in network. They didn’t even bill him until after the 2nd appt. Now he owes $400. Can he appeal? Did the doctor’s office meet their legal obligations?
