Please help ELI5 all these different “gaps”

Hi everyone, I am a software developer working at a new company in the health insurance industry. I have a business document explaining the requirements for a new dashboard doctors will use to treat patients. In the requirements section, I see the word “gap” used in several different contexts, but my google-fu is apparently weak today and I can’t make sense of what the actual differences between these “gaps” are…

“Patient risk gaps” “Patient screen gaps” “Patient quality gaps”

My inital google searches show that a “gap” is like a gap in coverage. For example, if you go to a doctor that is out of network, the “gap” is the difference in money you would pay for seeing that out of network doctor. Is that close to being right?

I appreciate any and all help from anyone willing to help me figure out all these different types of gaps. Thanks!
