I might have to switch to an Anthem PPO in CA where my specialty med has a quantity limit. It’s a very expensive medication ($15k) if I had to pay for the amount over the QL OOP.
Has anyone gotten approval and coverage for a specialty medication above the quantity limit? The medication already requires prior authorization just for the med within the QL.
