Seems causing excess grief and stress needs to be added to whatever pain you have before they will pay up.
6 months of appeal after appeal before they finally gave in.
Every year for 4 years straight this has happened for the same pain relieving procedure that they say is not medically unnecessary.
Lets inflict pain on the sub human head that does this.
I changed a job just to get away from the insurance coverage that my employer had.
