Are HMO’s really that bad ?

Recently moved to the US and everywhere I look people are shitting on HMO plans, but I don’t understand why ? In other countries, it is absolutely normal for your primary care practitioner to be the quarterback for your health. They figure out what tests/labs/specialist consults you need, and send you to that specialist. I’ve never thought, nor ever met anyone who thought, there was anything wrong with it.

But here in the US, this seems to be a big deal. People pay extra (significantly extra) just for the option to be able to go directly to a specialist, and I don’t at all understand why.

So my question is, what’s so bad with HMO plans ?
