Billed for procedure I don’t believe I had.

Recently I have an appointment with an ENT, I recall him using the typical scope to examine my nose. However, I at no point remember having an actual endoscopy done. Now, I could be wrong as the appointment flew by— but I feel as if that is something I would remember. Either way, I am confused as to how I can be billed for something I didn’t authorize to be done. For example, I know what I am getting into when I go to the dentist, say for a root canal or something because I am informed that’s what is happening, and expect a bill after they bill my insurance. They don’t even do X-rays before checking my insurance.
