Billed for someone else’s immunization I did not receive – what steps to take?

Hi folks,

I have a Cigna PPO plan from work. Randomly out of nowhere I received an EOB from a date of service a month ago of an immunization from a Walgreens. The insurance covered everything, but the bill has my name on it and I have not been to a Walgreens in ages nor received an immunization there. The EOB has my name on it and the Walgreens just shows a billing address in Chicago ( no specific location specified).

The only potential thing I could think of is early november I got a flu shot at my doc’s office at a one medical office, but that EOB was clearly laid out as my bill.

I called Cigna this morning, and we tried to contact Walgreens about this mystery charge, but we got telephoned around to 8 departments who didn’t help lol. So Cigna just ended up reversing that payment, and I’m supposed to await a response from Walgreens? The agent mentioned an updated EOB would be sent out but i don’t think it’s going to reveal anything.

Any ideas on how to proceed with this? Should I be worried about identify theft/someone using my insurance or is this a one off billing error? Just want to be sure! Thank you for all the help.
