I am getting a surgery in about three months And I need to get insurance that covers it. The plan that my husband’s company offers says that the calendar year deductible is $1500 individual and $3000 for family. It also says the calendar year out-of-pocket maximum is $5000 individual and $10,000 for family. But It says that for outpatient surgery they cover 80% after deductible. I have no idea how much my surgery will be because they’re not giving me a number. But based on my own research it seems like it’ll be above $30,000. Does the calendar your out-of-pocket maximum means that I will only have to pay $5000 for my surgery and then they will cover everything after ? Do we have to meet the family deductible or is it possible to just cover the individual deductible for me. Also, is it possible that health insurance plans do not cover rehabilitation services because on the catalog that my husband got from his work it doesn’t say anything about rehabilitation services so I am afraid will be left to pay the entire cost of my physical therapy.
