Claim denied – who do I contact?

In June 2019, I paid upfront for orthodontics ($3500) and was told my health insurance would start starting reimbursing me until I was paid out in full for my orthodontics coverage which covered $2000 and I was supposed to get the full $2000. I got about $400 every check. In January 2020, my employer insurance changed. I realized I was no longer getting checks from my old insurance so called to find out what was happening. They told me I needed to have my provider submit a claim to my new insurance, which also has orthodontics coverage, so my provider did so. I found out today that my insurance denied the claim. Am I SOL because my company changed insurances? Do I contact my dental provider, previous health insurance, or current health insurance?
