My employer changed health insurance plans starting July 1st. I had services in June and had coverage then. Insurance company is denying claims because the old plan is now canceled. The letter says because they no longer have access to the funds.
Can they do that?
It was in network, during coverage period, etc. All the right things, just really bad timing to need the services.
What words do I use to appeal and get them to approve the claim?
