Co-Pay and Co-insurance!?

I was treated at two different urgent cares recently. One was out of town, and in my insurance provider’s extended network where I have lower benefits. I had to pay a $50 copay and 20% coinsurance. The total discounted bill was ~$95. Of which I paid $50 copay and $19 co-insurance for a total of $69 of the $95 invoice. My insurance paid only $26!

My more recent visit was with a local primary network urgent care. My plan benefits there list 90%/10% with no mention of a copay. (Actually, it says 90% under CoPay but assumed that was really the benefit amount.) For the medical exam, there was a $35 CoPay and $7.84 Co-insurance the plan paid $35.59 on my EOB.

Everything I have read about CoPay and Co-insurance it seems like the insured pays one or the other, not both. Is it legitimate for Insurers to require both? Are there regulations for this?

I live in North Carolina.
