D-SNP vs Medicare Advantage

My facility is having trouble getting payment from patients with D-SNP plans from some of the most popular carriers. This is unusual as the same carrier would provide coverage for patients with just the Medicare side.

No one has been able to explain this to me. Why would a carrier who provides dual coverage for both a person’s Medicare/Medicaid deny a claim that they would usually cover if the patient only had their managed Medicare plan?
