Denied ED admission due to lack of pre-authorization

My daughter was being treated for a rash as an outpatient but the rash got significantly worse and our dermatologist told us to go to the emergency room for treatment of staph scalded skin syndrome. We were admitted from the emergency department and stayed for 6 days getting IV antibiotics. They think it was a bad case of eczema that got infected and not actually staph scalded skin syndrome.

I got a letter from our insurance company that all claims related to that admission are denied due to lack of pre-authorization approval…

I will be appealing this obviously, but what am I missing? How can an admission from an emergency department need pre-authorization?
