Dental insurance claim question

I apologize if this is just me being dumb, but I’m really confused and don’t really know how insurance works.

I had some dental work done two months ago. The price I was quoted for the procedure was roughly $600, and insurance would cover all but $120 of it. I had the procedure done and paid the $120 out of pocket.

Recently, I got a letter from my insurance saying that the claim of roughly $375 from my dentist had been processed, and insurance covered all but roughly $58 of it and that I owe the dentist $58.

How is this possible?
