My treatment plan said the following for the uncovered items –
Three Columns Estimated patient responsibility Estimated insurance responsibility And * UCR
The estimated patient responsibility column had figures in them which I paid.
The entire estimated insurance column is filled with $0
And the *UCR which I understand to be the usual and custom charges for the procedures.
Not surprisingly the insurance company declined coverage for the uncovered procedures. But I just want to make sure since they noted the estimated insurance contribution was $0, they pre authorized and already knew it wasn’t covered and can’t come after me for additional money.
