Do you have to pay off your deductible or just billed up to that amount?

I apologize for a silly question. I’ve had a lot of medical things this year and it’s really the first time in my life. Currently in ER

Im on a high deductible plan, pay (in theory) no costs once met. Say my current bills rack up to my entire deductible amount. Will no-cost benefits only apply once I’ve paid off the entire amount, or does simply being billed that amount allow those benefits to kick in?

I was thinking if doing a payment plan, but not if it won’t satisfy that requirement and allow the benefits to begin.
