Dr is in-network, but practice is not?

At week 6 of an 8-week severe and first-time migraine, I searched my PPO insurance for a neurologist and found Dr. J, who ran a ton of tests and also referred me to a pain specialist neurologist (who instantly diagnosed me with “occipital neuralgia” and “some sort of nerve/myelin issue”, also “in-network” but I’m paranoid now). I’m now getting notifications that the neurology office is not in-network, but I know Dr J IS in-network. This was her only address listed on the insurance.

I triple-checked: all doctors and imaging centers were in-network, I went to the addresses listed by insurance, all tests I’ve had done were pre-approved. I thought I did everything right.

How can a dr be in-network, but not the practice? If the practice isn’t listed on the insurance website (it only lists doctors), how am I supposed to know it won’t be covered? What are my options, if any? Where did I screw up?? Help me understand so I can avoid this in the future.
