Dr. Suddenly no longer in network (post surgery)

Age: 27 Zip: 55422 Income: $80K+

I had surgery and am dealing with the following:

  1. I scheduled my surgery with a provider who was in network. I have an approved claim with EOB stating they were in network on 3/2 (intake visit).

  2. My surgery was scheduled for 3/19. I am now seeing on my approved claims that this is being billed as out of network. Further looking into it, it seems like the Dr. is no longer in network on my plan.

I wasn’t informed at any point by either the provider or insurance that they were no longer in-network. Was I expected to check my providers network status every single day leading up to surgery? Do I have any recourse?
