Family of 3 looking for health insurance for the first time

I (30f, non-smoker), husband (31 m, smoker), and baby (6 months m), need health insurance, baby for obvious reasons but I’m looking at possibly having a surgery on a shoulder that has been injured for years depending on what the upcoming MRI shows. We are currently on Kentucky Medicaid (Pulaski County) but do not qualify anymore. For some reason their window for enrollment is tiny so I’ve got to figure it out immediately, but the cost is making my anxiety flare up so bad I screamed the last time I picked my baby up because my chest felt like it was caving in (this has not happened to me before) and I can’t think straight.

This is the quick compare sheet they gave us: https://imgur.com/Y2Y1OP3

If preferred, I’ll enter it all manually into the post. I looked at the ACA marketplace and they’re talking about $800+/mo premiums. Husband makes hourly rate of $11.50 for paperwork purposes, but he’s a delivery driver that gets paid by the piece and mile which get added onto his hourly as a “bonus.” This year their estimate is that his income will be slightly over $52,000. We do not qualify for assistance according to the Kentucky Kynect pre-screening tool. We’re struggling to pay the bills as it is, we just paid rent for August two weeks ago, and have to wait until we get our tax return to pay up the rest of the year.

Please point me in the right direction, because living is not looking affordable.
