Health insurance law?

Is my health insurance company required by law to tell me what they cover and don’t cover?

For example, my company denied a claim for acupuncture, saying that the it wasn’t covered for the condition specified by the practitioner.

However on my explanation of benefits, it is stated that acupuncture is covered in network.

I called my insurance company and they said there is no list of conditions. They won’t answer my questions and I don’t know what to do!

I’m considering reporting them for bad faith.

Is this illegal? Thank you for any help.
