Help deciding on employer plans

trying to decide between these two plans at work.

HSA pan seems cheapest in most cases. My spouse has medical conditions and sees a specialist about 6 times a year, and has blood tests about 3-4 times a year, has 5 generic prescriptions, and seems t end up in the emergency room or urgent care every few years, or needs some procedure like an endoscopy or imaging.

The only way I see the PPO saving money is if she and only she meets her OOP max, and that rarely happens.

The way I see it we would need 20+ doctors visits to make the co-pay be a better deal.
