How did my health insurance company know about my prescription?

So a couple of weeks ago, I sought a psychiatrist for mental health issues. I didn’t want to go through my insurance because I didn’t want this information on my medical records (mostly out of embarrassment and privacy), so I paid completely out-of-pocket for this appointment. This doctor never asked or received any health insurance information from me. The doctor wrote and sent an electronic prescription to what I believed was a small “independent” pharmacy. I paid for the medicine at the pharmacy and was never asked to give my insurance information. However after I received and paid for my prescription, I logged into my health insurance website and saw that there was a claim for my prescription. This is horrible because I did not want my health insurance to know anything about this issue and I’m not sure how they knew I got a prescription (I never submitted insurance information to the pharmacy either). I asked the psychiatrist who saw me if he somehow got a hold of my insurance information and then sent my insurance any information about our meeting or my prescription and he said he didn’t. Is there any way to remove this information from my health insurance records? How did this happen?

Any help anyone can give me is greatly appreciated!!
