How does health insurance work when your are working a 100 percent remote job from another state?

If a company’s headquarters is in Washington DC for example, but you live and work 100 percent from home in Texas and the job is claiming to offer health insurance benefits, how does this work? Would it most likely be a HMO or could it be a PPO? Also, how does it work for a domestic partner or spouse of the person working remotely? Can your spouse be put on the plan as well? Thanks
