I live in NC. I have insurance through Cigna; $250 deductible; $2000 out of pocket max. I had a surgery 10/28 and Cigna was billed $28k. The claim was not fully processed until 11/30 at which point I hit my deductible and out of pocket max. Between surgery day (10/28) and claim processing day (11/30) I had several therapy visits and other doctor’s visits who’s claims processed within days. My question is, is hitting your out of pocket based on initial claim date or claim processing date? Can I get those in-between visits re-processed so I don’t owe? I’m not sure how all of that works.
