How likely is it that insurance companies will begin canceling/refusing coverage for people who are not vaccinated against COVID?

I have my own opinion on whether they should, but I’m curious if this is actually legally feasible looking towards the future. If it’s possible for an insurer to refuse to pay for COVID treatment undergone in hospital by someone who refused the vaccines. Right now I’m only aware of insurance offering incentives for vaccination and things like quitting smoking.
