How to know my insurance won’t deny a procedure?

I have a small basal cell carcinoma near my eye. I’ll have it removed by a dermatologist using MOHS surgery. Since it’s on my lower eyelid I want a plastic surgeon to close it. I already did a consult with the plastic surgeon who agreed that if it’s not closed properly it could affect the function of my lower eyelid.
I’m confident my insurance will cover the removal but I’m concerned they’ll say that the plastic surgeon wasn’t necessary and I’ll be stuck with a bill for thousands of dollars. At a dentists office a couple years ago I trusted them when they said everything was covered but then afterwards they said the insurance didn’t cover everything and I owed another $1200 so I’m leary of just trusting the Dr. s office. What advice do you have to be more certain that insurance will cover it?
