I forgot to fill out the Health Insurance questionnaire about an injury, and now I got a letter that they won’t cover any costs. Is it too late, or should I call and fill it out?

I got the questionnaire in the mail and forgot to fill it out and now they sent a letter saying “No response for additional information requested”.
