I will need a hip replacement soon but I have no insurance and all STM seem like they don’t cover anything at all

I’m 31 and injured my hip at the beginning of 2020 while working out. Because of the pandemic the injury wasn’t too extreme but once I started work back towards the end of 2020 it got worse (I walk 3-6 miles everyday). I went to an urgent care in GA to get an x-ray middle of last year and the doctor said I’d need to have a FHR to completely fix the issue but didn’t list that on my summary sheet, only “hip pain”. So my questions:

Would this be considered a preexisting condition even though I’ve never been treated for it?

Most STM list “no joint replacement unless caused by injury” would this be considered caused by an injury? Or does it have to be documented somehow?

Any and all advice would be greatly appreciated. It’s getting harder and harder to walk and therefore work, I’m afraid eventually I will not be able to work or no one will hire me due to my handicap so I need to take care of this asap. Thanks
