Imaging claim processed under me (parent) instead of my child (toddler) – What further steps do I take?

I’m hoping this is a pretty easy question to answer. My (under two) child had an ultrasound imaging appointment (at a children’s imaging center no less) and I provided them with all of my child’s information with regards to their particular member ID, etc. They even scanned the member ID card.

I just received the EOB and I was wondering why the estimated portion to pay was high when I knew my child had met over 60% of their deductible last year (DOS was December 2020). Began comparing other EOBs to this one and finally realized after an hour that it was sent and processed to insurance under my name, not my child’s!

Whom do I contact, the insurance company, or the imaging center? I am assuming the imaging center so they can re-send the claim but just wanted to come here and be sure so I don’t waste my time with them. I live in Texas if that helps.
