Insurance Billing Denied despite Coverage (CO)

I had a daughter born in the middle of October. She had to be hospitalized around the 20th of November. Billing was done separately to two separate outfits, children’s hospital was easily submitted and paid for by the insurance. The group that supplied the doctors has billed Anthem Blue Cross twice now, and the doctor group keeps saying every time they contact Anthem they are told she’s outside the 30 day grace period. However, she was added to my policy effective her day of birth, I even have a health insurance card stating that and called anthem to confirm that. The doctor group have confirmed that have the correct info, I have sent them an image of the card, and told them I have called Anthem and there should be no issues. When I look at my claims online on Anthems site, there doesn’t even show it ever processing, denied or otherwise.

They are attempting to bill a 3rd time now. If that fails, how can I escalate this to get them to fix this issue as there should be no reason its failing? Is there some sort of reporting agency I can threaten them with? I feel like someone with a brain in their billing department needs to call anthem and get it sorted, but they are big and seem to just want to push it through standard channels.
