Insurance provider asking to be repaid from a three+ year old procedure.

I live in NY, and recently received an invoice from BCBS MN asking that I repay them almost $2,500 that they had paid for anesthesia I got during a procedure in December of 2018. They included an EOB showing that amount moved from “allowed amount” to “patient non-covered amount” with no explanation as to why it’s no longer covered.

I called them to find out what was going on; the rep was unable to give me a reason why the anesthesia was no longer covered, but did advise me that this invoice was generated as the result of an audit and wasn’t sure if I’d actually have to pay it or not.

I just received a second invoice requesting the funds, which unfortunately I don’t have as this is totally unexpected. Can they really ask me to repay them 3 years after the fact?
