Is there any discretion with coding?

My gynecologist urged me to get a transvaginal ultrasound to see placement of my IUD after I had abnormal bleeding. All of my previous women’s health appointments, pills, and devices have been covered in the past, under United Healthcare. I did ultrasound done under the impression it would be covered. Also referenced the verbiage on a flyer from my insurance: “Place, fit, inspect or remove IUDs (intrauterine devices)”. Well it wasn’t covered according to my dr’s office.

Spoke with United Healthcare who said it is all up to medical coder to input it correctly. Spoke with dr’s office who said she would push it higher up for review–never heard back. I’m in my 20’s and still learning so my question is: who is person that reads my healthcare coverage and codes something as covered/not covered? Any other advice or do I take a loss on this one?
