Is this good health insurance, and how do I interpret the costs?

Is this good or bad coverage?

Also, I don’t quite understand it — it says 100% preventative is covered, but PCP visits are $35. If PCP visits aren’t preventative, what is considered “preventative”?

And it says specialists $70, but also either 80% or 50% covered depending on in-network. So if you see an in-network specialist, is the cost $70 or is it 20% of the total cost?

Does anyone know if therapy falls under any of these categories and if you can tell from this how much it would cost?

And am I understanding properly that if you see one out-of-network doctor, e.g. an out-of-network therapist, you have to pay $7,500 out-of-pocket to meet the deductible? If you do that but also have an in-network PCP, have you also met the deductible for the PCP, or do you have to pay a separate $2,500 deductible for the in-network doctor?
