Is your health insurance provider obligated to tell you if they won’t cover your medical charges?

I used my medical insurance to pay for a doctor’s visit. 18 months later and the doctor’s office is contacting me for outstanding claims because apparently my medical insurance never paid up. But I was never notified of that? Shouldn’t my medical insurance have sent me a bill?

This is getting me into a lot of trouble now. I’m wondering:

(1) What I can do now to right the situation (I think it’s unfair this visit wasn’t covered at least in partiality, so I want to negotiate.. but at this point it seems like I don’t even have time to negotiate, which also seems very unfair.)

(2) What I can learn from this situation. Should I have done something before? Was it my responsibility to follow up on this kind of stuff? If so, isn’t this part of what I pay insurance for? Shouldn’t they be the ones following up on these things?

The most frustrating thing for me is that I feel like someone should have told me nothing was being paid! This is 18 months after…and all the while I had no clue this was still outstanding. And now I’m being treated like I’m some kind of criminal, with the rate the office keeps hounding me.
