MA, USA. Are RXs covered based on fill date or pick up date?

I had a 4 or 5 day gap in health insurance as I changed jobs recently. I didn’t have the new health insurance card when I picked up my prescriptions, so I payed out of pocket using my HSA.

I saved the RX receipts/fill slips and the receipt showing the date they were actually purchased. I intentionally waited until after I knew my new coverage was active before picking up the RXs.

I submitted 3 RXs for reimbursement and received deductible credits for 1 of 3. The one covered was “filled” after my new health insurance started. The 2 others were “Filled” during the gap, but were picked up and paid for after my new insurance started.

Do I have grounds for an appeal? If I had known this would be an issue, I would have asked the pharmacist to un-fill and re-fill the prescriptions.
