My Doctor filed a claim for a service that he refuses to provide ? What do I do ?

I took my son to a specialist for a psychological assessment . In September of 2020, I had an initial consultation and was told to come back for a more extensive fact finding appointment. In early October, we did. We spent 4 hours with the physician at this time. He gave us some surveys to complete and told us that we should expect a written assessment in 2 weeks time after the surveys are returned.

We returned the surveys, but did not hear back from the doctor. We waited patiently for about 6 weeks. At this time, I sent in a series of emails to follow up on the missing assessment. The office acknowledged that they had not sent the follow up – and forwarded my message directly to the doctor. The emails were not answered. I also did not a receive a phone call.

A few days later – which would be late December – a bit prior to the New Year. I called the office again. At this time, they said the doctor was on vacation and could not be reached. I let them know that they had failed to deliver the service and asked them to confirm that they had filed the necessary claims for the service. At that time, they responded by stating that the assessment would be delivered by Jan 4th ( a couple of weeks later). On Jan 4th – I did not receive nor did I receive any feedback from the office but I did notice that they had billed for the final assessment delivery at that time.

When I reached out for a final time they re promised the product to be delivered in hand by Feb 1. On that date (yesterday) they called to say that they did not have it again.

At this point, I feel that our trust with the provider is broken due to extreme disregard. I do not wish to even receive an assessment from this provider. What should be our next steps ?
