Navigating elusive health insurance benefits…

So my doctor said I need to have surgery and I am trying to figure out what will be covered by my insurance (United Healthcare), and more importantly, what I will have to pay out of pocket. I have a $0 deductible and $3500 max out of pocket which, being the beginning of the year, I haven’t touched. My copay to the hospital is $400 but other than that I have no idea what to expect.

I spoke to a representative at UHC and they told me it would be covered 100%, but their online estimator said insurance wouldn’t cover any of it…. so I talked to another person at UHC and they said they would need the surgery code in order to give a quote. I called the doc office for the code and she said she could run it for me and said that it would be $3500, my full max out of pocket. I can’t find any info in my plan documents about what my co-insurance is either.

I’m going to get in touch with UHC again to try to get some clarification but thought I’d ask here in the meantime…

Anyone else run into a similar situation or have any insight into how this process works?
