On medical leave from work, might lose insurance and job when FMLA runs out. What next?

I took a medical leave from work and since I quality for FMLA I was able to secure my job with the 12 weeks (unpaid). I’ve been seeing doctors and paying for my visits through my company’s health insurance. My company also has a private disability insurer but I expect them to deny my claim and not receive any pay.

Once the FMLA runs out, I will likely be fired, since I am still seeing doctors and am unable to return to work.

What happens next? I understand with COBRA I could elect to pay the full cost of my health insurance for the rest of the year but in all likelihood that would be too expensive for me. My company’s health insurance premiums are $150/month, but the full price for the year is probably closer to $1000/month (my guess).

Does that mean my next step, assuming I’m unemployed and still in need of medical care, is to apply for Medicaid?

Do I quality for any unemployment benefits? I understand these are normally for people who expect to find work.

Additional context: State is New York. I live with my parents, am 18+, and I only worked for about a year. I have some savings, which I think can affect my eligibility for these programs.
