PPO or CDHP as a person with a chronic disease?

It’s my time picking out insurance and dealing with premiums, deductibles and copays since my previous medicaid covered everything with 0 cost. Anyways, I’m picking between PPO or CDHP offered by my employer.

So I have a chronic disease for life. Nothing too major, I just have to do checkups with a specialist every 6 months and taking meds everyday. These checkups will lead into getting blood work, ultrasounds, bone density scans. Long story short, I get healthcare more than the average person. With that in mind, I’m guessing I should go for the PPO plan since Annual Deductible is $500 lower and I get more coverage (90% vs 80%). It also seems like I will only have to do a small copay instead of paying 20% for pcp and specialists. The PPO plan is also only $10 more than the CDHP. So in total, including the little adds on I’m doing like FSA contribution, Dental, Vision, it comes out to $347 a month. Hope my logic is correct here.
